DS Public Safety Coordinator

Community Care Network is looking for a Developmental Services Public Safety Coordinator to join our team!

Community Care Network is comprised of Rutland Mental Health Services and Rutland Community Programs. Our mission is to enhance the well-being of our communities, individuals and families through responsive, innovative and collaborative human service

About the Role:

The Developmental Services (DS) Public Safety/ACT 248 Coordinator plays a key role in supporting individuals with developmental disabilities who are involved with public safety systems. This position emphasizes the promotion of individual and public safety, crisis prevention, and successful community integration. The Coordinator ensures that supports are in place for individuals receiving Developmental Services under ACT 248 or a public safety funding source.

As a central liaison between internal teams, external providers, law enforcement, hospitals, and community partners, the Coordinator ensures collaborative, person-centered care. This role requires strong organizational and communication skills to uphold the individual’s conditions of release and to deliver services aligned with their person-centered support plans.

The Coordinator provides both direct and indirect support to help individuals achieve meaningful quality of life outcomes, ensures compliance with required documentation, and mitigates public safety risks and ACT 248 violations.

Responsibilities:

  • Complete all required documentation accurately and in a timely manner, including but not limited to:
    • Individual Support Agreement Attachments (Behavior Support Plans, Community Safety Plans, Communication Plans)
    • Wellness Plans
    • Home Visit Notes
    • Monthly Summaries
    • Contact Notes
    • Critical Incident Reports
    • Sexual/Violent Offender Intervention Treatment Progress Scales
  • Serve as a consistent point of contact for communication and collaboration between internal teams, external providers, law enforcement, hospitals, and other community stakeholders.
  • Support crisis prevention and resolution, including problem-solving around issues such as housing instability, medical concerns, team disagreements, and violations of court orders or conditions of release.
  • Provide direct support in the field, including:
  • Coordinating housing transitions
  • Building community relationships
  • Supporting Home and Community-Based Waiver services
  • Providing transportation to medical and therapy appointments
  • Conduct daily check-ins and ongoing support with individuals on assigned caseloads to ensure services align with their needs, preferences, and legal requirements.
  • Refer individuals to appropriate community resources based on personal or guardian requests.
  • Maintain compliance with mandated reporting laws regarding abuse, neglect, and exploitation.
  • Uphold professional boundaries while working collaboratively with individuals, teams, and other stakeholders.
  • Other duties as assigned.

Qualifications:

  • Associate’s or Bachelor’s Degree in a related field preferred.
  • Qualified Developmental Disabilities Professional (QDDP) credential required.
  • Direct experience with Developmental Services, Public Safety, and ACT 248 is required.
  • Strong written and verbal communication skills.
  • Excellent organizational skills and ability to manage multiple priorities.
  • Ability to work independently and exercise sound judgment under pressure.
  • Commitment to maintaining confidentiality and following agency and state policies.
  • Flexibility, reliability, and a positive, team-oriented attitude.
  • Valid driver’s license and reliable transportation with automobile liability insurance meeting Vermont state requirements.
  • Successful completion of a criminal background check post-hire.
